Check in order: the account's cadence - if the last run was inside the window (default 7 days), it isn't due yet; that the account is active in Clicc; and that the campaigns you expect are tracked - only tracked campaigns are analysed.
Runs occasionally fail on temporary Google Ads connection issues. The next cadence pass retries automatically; a failed run never applies anything.
Almost always a cooldown - the decision card shows which window is active and when it expires. The other case: a target adjust on a portfolio campaign, which Google requires you to make on the portfolio strategy in Google Ads directly.
Read the card's reasoning first - it names the baseline it judged against. Nine times out of ten the surprise is a stale account Target CPA or a campaign tCPA nobody remembers setting. Fix the target; the re-analysis re-judges everything.
The analysis window (default 30 days) is the judge - historic conversions outside it don't count.
